Medical Social Worker

• Assessment of patient’s emotional, environmental, financial, social, and support needs.
• Counseling Assistance
• Communicating with the patient’s health care team and physician to report the patient’s progress, concerns, needs, and possible problems.
• Financial Assistance
• Placement Assistance
• Provides social work services in the patient’s home, including crisis intervention and community resource planning.
• Works closely with all members of the patient’s health care team in the development of a care plan, including discharge plans.
