    This team works closely with the patient’s primary care providers and specialists to ensure continuity of care.

Who Is Eligible?

  • Patients with a terminal diagnosis and a life expectancy of 6 months or less (if the illness follows its natural course)

  • Individuals who are no longer seeking curative treatments

  • Patients in any setting—home, facility, or hospital. No homebound restriction required.

Who Is Eligible for Hospice?

You or your loved one may qualify for hospice care if:

  • You’ve received a terminal diagnosis with a life expectancy of 6 months or less

  • Curative treatment is no longer effective or desired

  • You prefer comfort-focused care at home or in a familiar setting
    Hospice is covered 100% by Medicare.
